Fill out a Public Records Request form or use your own paper and fax, e-mail, mail, or telephone us with the following information:
  • Your name, address, telephone number, e-mail address and fax number.
  • The date of your request.
  • Specific nature of your request to include:
  • A description of the requested records (title, subject, date, policy, account #, etc.)
  • Inspection # if applicable.
  • The number of copies you need and the correct mailing address(es).
